Of coz, that keeper has to go out. Then the team will down to 10 men with no goalkeeper. Substitution has to be made, 1 player has to go out, to allow a new keeper comes in to make it 10 men with a goalkeeper.
Usually , the midfielders will do the sacrifice

Yeah, they can get red cards. I remember one game in which the goalkeeper was removed and one of the other players had to take over (they couldn't replace with another goalkeeper because they used up all Thier switches).
That player thought the whole thing was really amusing until he got bombarded with goals. It was a sad sight.

I remember 2 specials: World Cup 94, Italian goalie Pagliuca got a red against Norway, Italian Coach Arrigo Sacchi replace Roberto Baggio with the goalie Marchegiani.
UEFA Champions Cup (not League, it was November'76), Borussia Moenchengladbach - Torino, Italians have 2 red and goalie Castellini get another red! All replacement are done, so Centre-forward Graziani replace Castellini and become that day's hero with 2 great saves (match end 0-0).
Today Graziani is a coach and is coaching a minor league team, Cervia, wich is filmed all the day by a troupe for an odd TV project - see the link http://www.tvblog.it/categoria/campioni

Any player can be sent off. In such circumstances where the goalkeeper is sent off, it is normal practice to withdraw an outfield player to bring on a substitute goalkeeper.
Where this is no substitute keeper (or if all 3 allowable changes have already occurred) an outfield player goes in goal.
