If it's not that big of a deal, as in my answer wouldn't really affect them, I'd tell them what they want to hear. But if it's something serious and they're looking for real advice, I'd answer honestly. It really depends on the situation.

Just had that situation this weekend. My friend is in a bad marriage and feels lost and trapped and asked my opinion. I was honest with her but also listened and was gentle with my answers. I'm always honest with my friends. But sometimes I am more careful with the answers if it's a delicate question.

If you can't go to your friends for the truth then who are you supposed to go to? Always be honest with your friends. As you said, it may not be what they want to hear, but they will always appreciate getting honesty from you. Let them go to the syncophants for false flattery...

I am not truly honest if it will hurt their feelings.

And if they prod me, I will sugar coat it with a few candy coats with a tiny bit of honesty.

Co workers...I tell them like it is like Dr. Phil!
