I have a baby shower to go to tomorrow, I forgot to pick up a card and all I have is a blank one. I really don't like the parents, I'm only going because she's a co-worker. Any suggestions on what I should write on the card?

Well if she is just a co-worker then you are not obligated to go. But it would be proper etiquette to do so if you work in close proximity in order to keep a good working relationship with her..

But you must at least know her/them since you stated that you do not like the "parents" which does mean that you know them on some level?

For whatever reason you are choosing to go to a "baby shower" for a co-worker that you do not like? I would suggest that you NOT go.

But if you feel some motivation to attend her shower then you should do so with all respect to the occasion.

A "blank" card can work both ways. It is all in how you approach the message. It can either offer that personal touch from a dear friend or the obligatory gift from a co-worker.

Simply write a brief message of congratulations. Do not over do the "how wonderful" thing, thus maintaining the distance you wish to maintain.

Write: Congratulations on the new baby, then sign it.

What's your bottom line? Overlooking the fact that you don't enjoy her company, what are your sincere wishes for any human being as they begin this stage of life? That this will be the most wonderful time of joy and discovery they can hope to imagine? That it will forever alter their appreciation for what it means to be a human being on this planet? Or simply for a healthy & happy journey throughout their new life together? Sincerity is always the best option, it is easily recognized, touches hearts, and helps us to see past our own comfort levels to what we really value, thus is affirming all the way around. You end up growing just a bit yourself every time you rise to such an occasion, and that kind of grace is both beatuifull and maturing.You can do it!
