You already have a good start just by being aware of things you need to change. The best way is to ask God to help you and make you into the man He wants you to be.

Priotize your goals from 1 to whatever. Take the first goal and ask yourself how you would go about attaining that particular aim. Then put all of the ways down on paper and take the first 2 or 3 ways and implement them in your daily walk. Soon you will be doing those things that you feel would make you the person that you would like to be. Then as you attain a goal, you will find that other goals that you at first thought were impossible or unattainable have been reached through the actions that you have already implemented.

Focus and do well on whats important to you. A good education is extremely important -- what I would do is sit down and say, "exactly what do I want to accomplish?" Make a list, and be specific of what you want to change to be. Then from there you can work out the steps it takes to get to that place.
But I'd say, more than anything, embrace who you are. You don't need to completely change. If there are things about you that you don't like because YOU dont like them, or know that they are wrong, then its alright to change for the better. But if you are wanting to change for the eyes of all the others around you, to impress and feel belonging, I would think very hard on it. Do you really want to?

You are headed down the right path, you have conquered the first step already, WANTING TO CHANGE!! Congradulations!
Now it is time to set goals for yourself, take baby steps though. Changing yourself can't happen in one day. Start by writing what you want to change about yourself. Then write a mission statement that you can eaisily put in your wallet/purse, tape to your mirror or put somewhere you will see many times a day. Example: Today i will eat at least one healthy meal. or Today i will through out some of my old clothes that i don't want anymore and shop for a few items that i would like to try. Start with small things, like getting a routine going for yourself day to day, Then work your way up to the bigger things. Changing yourself takes COMMITMENT and the will to do it. you cant just start one day and then take a few days off. It will never work. Just stick with it and you will succeed. Good Luck!!

First step : Know yourself
Second step : What you really need to change and not just want to change
Third step : Set up goals, not too high, so you can make it
And in between, write down what you feel, what you did, what you should have done but didn't, and be honest with yourself, what went wrong.
You could be the person that you want to be, if you have the talent to turn your personality in a positive way.

Just take it one at the time. Change can be hard, but in your case seems that you are really serious about it. That is a good start. You have to prioritize, start by the most important to you.

Work hard in school, go to college. Do a job you like and are good at. Always do the best you can. You will be amazed at how far that can take you.
