If a parent HAS NEVER exercised thier visitation and has had NO other form of contact with a child, can parental rights be terminated in Texas? If, so how and on what grounds?

The court will have to determine it's in the best interest of the child. While the parent obviously has not been a part of the childs life the court can't use that to terminate parental rights alone. Unless the parent is served notice to them in person they'll be able to contest any action taken even if they hadn't shown up in court.

contact a lwyer. But the parent may have to be given a written warning stating if they continue this pattern then rights wii be terminated

you will have to file a motoin in the court where the child custody case was done, the other parent will be served and given a chance to defend the motion.

If they have not visited, not paid child support and had not contract that should be enough

Only if he or she has ever been a threat to the child, ex: drugs, abusing, lawbreaker etc.
