If your original records were shreaded, then there is no way to get a copy. What you can have done is a titer, to see where you stand and if the vaccinations are still effective.

well i have to say this is funny to find out this happens to other folks, my files were shredded to and when i went to apply for my CNA i had to have a shot record and i have no records, so i went down to the health department and got my shots, i had to start from scratch, i think it is a total of 6-8 months worth

Find all the shredded paper and stick it all back togther like a massive jigsaw???? Or maybe they have your records on computer. If not I am afraid it is down to hard thinking and use of your memory .............. or start all over

Have they got in on a database?
I went for a new patient health check at my surgery and as well as having my original file they also have all my notes on the computer.
