Pill is very effective, like 98%. Doesn't make sense to get pregnant twice on the pill. Either she isn't taking it regularly and misses some pills some days or she is taking meds (like antibiotics) that interfere with it.

Yes you can. I was on the pill for 8 years and took it everyday like I was suppose and ended up pregnant. No contraceptive is 100% effective.

Yes. Oral contraceptives don't have a 100% pregnancy prevention guarantee. There's a 1-2% chance that a woman who is on oral contraceptives and who is taking them exactly as prescribed will get pregnant, and that rate is even higher for women taking a progesterone only pill or women who forget to take them and who then double up on their pills.
The only way to prevent pregnancy 100% is to abstain from intercourse.

I have a friend who got pregnant twice on the pill, once on the IUD, once using a condom, and them had her tubes tied and got pregnant again. She never wanted kids and is only raising one of her kids, but I never seen a women with such bad luck as that. But its true no method of birth control is 100%.

Maybe she did not follow the directions and that the pills for 7 days first without having sex, she most likely just took one pill, then had sex. Most people refuse to read the directions on anything.
I bet it's her own fault, not the pills lack of working here.

No birth control is 100% effective. The pill is the closest at 99.8%, but she has to be really unlucky to get pregnant twice while on it.

Yes, the only 100% way to prevent pregancy is 100% abstinence. The pill is very effective, but it is not perfect.
Also, the pill cannot prevent STD's.
