Mark Wohlers has thrown 103 in Spring Training, Nolan Ryan does have the official record by Guiness Book. But that was clocked by Guiness. They are not at all games.

In the 1940's Carl Hubbell threw three pitches through a US Army timing trap (a small window). The average of these three pitches was 98.6 mph. Carl was probably throwing over 100 mph if his average was that high, and to throw through the trap he probably took some velocity off of this throws.

FYI: The terminal velocity, if you drop it from an air plane is 140 ft/sec or 95.45 mph in free-fall.

Also it depends on where you point the radar gun. A ball loses roughly 10% of its velocity between the mound and home-plate. I have seen pro-scouts point the gun at the ball as it crosses home-plate just so they can say a pitcher doesn't throw that fast. Then when they sign him they point the gun at the point of release and say he is throwing 9 mph faster.

Guinness record is Ryan's throw, but many, many pitchers have been unofficially recorded throwing harder than that. J.R. Richard, a former teammate of Ryan's reportedlly threw as high as 102. There are a handful of closers today that can reach 100mph as well.
