kereoke is fun. twister is fun (especially since you havnt played since u were a kid,, and its a bit raunchy as an adult) u better get some good dancing music and get the place jumpin. just make sure that nobody messes around with the songs (ie, dont skip songs in middle,, make a playlist and dont stop the music) cuz that will kill a party fast. and dont turn on the tv, unless you rented a funny movie,, have an even boy/girl ratio, or try your best to have one, cuz it can be wierd when theres 2 guys and 10 girls, and 2 girls and 10 guys,,, just have fun

Have his favorite foods, a cake, and maybe some games that
are suitable for an 18 year-old guy. But after the "Surprise" let
him decide what goes next.

Maybe you could do a 'This is Your Life' kinda of thing. Have pictures of him from the time he was a baby until now? Maybe fix up a collage of pictures of various times in his life? Maybe you could have his friends and family tell funny stories of stuff he's done or stuff they've done together? Maybe fix up a book of memories for him?
I could go on and on but hopefully, this will give you some ideas. :)

Hire a D.J. - Danceing
karaoke- Singing
Water balloon fight. Sounds childish but it's fun.
Have a cook out, go to the lake. Play vollyball, horse shoes ect.
Perhaps paint ball

it depends how many people are there
you could play some kind of fun game that gets every one involved and makes everyong laugh and losen up
probably a game that is revolved around your friend and embarrassing him thats always fun
not a game like monoply something like pictionary or cherades people tend to like pictionary or something like that rather than having to get up in front of everyone and look like a dummy sounds kind of dorky but once everyone gets involved it will be fun
you could always make only your friend do the drawing and guessers earn points for a chance to do something to the birthday boy like put a pie in his face something everyone will want to work for and get involved with

Well, you need balloons, games, invites, drinks, food, alot of preparation is needed for suprise parties, speeches, toasting, cake cutting time, dance music, lots and lots.
