Put the tip in the tip line of your credit card receipt when you're signing for it.
You can always just tip him/her cash and leave the tip line blank.

i prefer to tip in cash to the waiter. when tip is added in credit card payment, i sometimes wonder if the tip really reaches the waiter or is kept by the manager on the counter.

Either at least 15% on the credit card receipt, or if you really want to be nice...give them a cash tip because they don't have to pay taxes on it!

fill in the tip line. after that evening, someone at the restaurant runs the credit card authorizations and the final amount is charged. (i believe that is how it works)
If you like the person, leave a cash tip.
