In my personal opinion, the American Civil War was the worst war the USA has ever participated in. One main reason (other than the fact that it was brother against brother), was that at the time, weapon technologies were advancing at an ever increasing rate but generals were still using Napoleonic tactics. This led to huge death tolls, along with the lack of medical advancements such as antibiotics and even simple sterilization practices like washing hands before and after surgeries. So many lives were lost that could have been saved say 50 years in the future.

The Battle of Antietam (Sharpsburg) still holds the record for the single most deaths in one day of battle. The Battle of Gettysburg - a three day affair - also holds the record as the most deaths in any one battle.

Economically speaking, the Civil War was catostrophic, mostly for the Southern states. It took roughly 100 years for the South to fully recover.

Actual fighting during the American Civil War lasted about 4 years, but that doesn't even include the years of mounting tensions before the war and the many years of tense Reconstruction afterward. As a result of the war, the leader of the country was assassinated. If I'm not mistaken, Abraham Lincoln was the only President to be assassinated during wartimes.

The worst as far as cost and destruction for the US was the Civil War by far.

It lasted 4 (1861-1865) years and cost over 500,000 lives. In fact, there were battles in the Civil War where more Americans were Killed in a single afternoon than we have lost in all the time we have been in Iraq.

WW II cost far less in total deaths in a larger population base with far less damage to the American homeland. It really took the South a hundred years to fully recover from the Civil War.

World War II only lasted 3 years 9 months from Pearl Harbor to VJ Day. For total loss of life, cost and total damage it is the worst war.
