The Final itself, in 1994 (Brazil v Italy, 0-0 after 120 minutes, Brazil winning 3-2 on penalties)

The shootout was first introduced to the World Cup in 1982, and the first game to be ended in that way was France v West Germany (3-3 after 120 minutes, West Germany winning 4-3 on penalties)

The first and ONLY time the Final game was decided by penalty kicks was in the 1994 USA World Cup.
Brasil - Italy 0 - 0 after reg. time and 3 - 2 after pks.
