I'm not sure what you mean by "what happens" on the inside.... the doctor stitches each layer back together and puts everything back where it was. You heal up and everything is pretty much the same as it was, except that your stomach muscles have been separated and it takes forever for them to have any strength again, and you'll have that lovely "smiley face" scar across your stomach forever.

The incision or cut heals and there is scar tissue or tissue that isn't as strechy as before.
