When she wakes up from the coma she looks at both her hands and says "four years"!!!! How does she know, at that precise moment, that it was 4 years?

I think that we are supposed to believe that she could read her life line on her palm and know that she has lived 4 years of her life.

It have been a hint that she knew how to read her palm or what's called palmistry. Palmistry is found in many ancient cultures and is used for not only telling life events, past present and future, it was also used as a medical tool to determine health factors such as blood flow, respiration, infections, etc. Each line, finger, joint and area of the hand corresponds to a part of the body and an area of the body. She probably would have learned it from Pai Mei when she learned his martial arts.
There are patterns that mean certain things such as time of bad health or imprisonment or just hard times in a persons life. Palmistry has gotten a bad rap over the years since its association with the occult since it does share some traits from the systems of ancient astrology, numerology and Tarot among others but plenty of carnival type 'Psychics' and sharlatans have used it to dupe the gullible. It is a skill that can be learned and can be a great window into understanding yourself as well as those close to you or even complete strangers. I learned it in my 20's out of curiosity of things I didn't understand. You have to practice to get good at it but there is no mysticism in it whatsoever. It is no different than reading a map. Beatrix more than likely would have seen a time of bad health and deterioration of health.

she wasnt completely gone in her coma, she was able to remember stuff that occured while she was in it. Like the guy sayng shes been under for 4 years.

I never fully understood that myself, but it does a close-up of her hands just before she says that.
So I just assume that she looked at her hands and could tell how much she had aged by the wrinkles.
