The Church teaches that Marriage is a sacrament, and in the Roman Rite anyway (as opposed to the Eastern Rite, also legitimately Catholic in every way), the Sacrament of Marriage is conferred by the man and woman entering into the marriage. The priest and two witnesses are just that- witnesses. The priest gives blessings, but he does not confer the sacrament. The sacrament remains until one person in the couple dies.
What the Church is looking for in the case of a Church marriage is whether or not, at the time of the scarment wa given and received, each party had the intention of keeping the vow to remain married until the other died. There are a variety of reasons why such a marriage would be invalid, and therefore not a scarament, such as immaturity, perjury (lying about keeping the vow), etc. This type of annulment involves a tribunal, which investigates the case, much like a marriage court, and makes a determination. In the United States, Austrailia, and Europe, these cases can take anywhere from 6 months to 2 years.
Ms. Kidman married outside of the Rite of the Church. None of these fit Ms. Kidman's situation.
A Catholic who has NOT married according to the Rite of the Church has not received nor imparted the Sacrament of Marriage. However, the Church assumes that ALL marriages contracted are valid, until proven otherwise. Therefore, there would need to be a decree of nullity.
HOWEVER, because Ms. Kidman's marriage to Mr. Cruise was NOT according to the Rite of the Church, all Ms. Kidman would be required to do is prove she did not rectify the situation by having the marriage convalidated (doing it over again according the Church Rite). This is, in most places, done by a declaration of the party attesting to the fact (Ms. Kidman) as well as 2 witnesses who declare the same, and those declarations are notarized by a priet or other Church official. These declarations are processed through a tribunal- usually in rapid succession (30-60 days) for ANY party who contracted such a previous marriage, not just celebrities. The party is then free to marry.
The Catholic Church, contrary to popular belief, by the way, *never* calls ANY child legitimate or illegitimate if the parents are involved in an annulment (also known as a decree of nullity). For that matter, the Church NEVER calls a child illegitimate who is born without its parents being married. The fees involved cover a complete court hearing, and are waived or set in payment plans for those who cannot afford them. A Catholic who is divorced but has not contracted another marriage outside of the Church may receive Holy Communion and fully participate in the life of the Church, provided they are in a state of grace (been to confession and received absolution about having contracted a marriage outside f the Church, etc.).

Nicole is a Catholic. In answer specifically to the first part of your question, if you are Catholic and you marry outside the Church your marriage is considered invalid. Therefore, if you get a civil divorce and go get married in the Church that is a first marriage sacramentally speaking.
I don't know where Nicole and Cruise got married, because she had to get an annulment, which is where the Church studies the circumstances of your first marriage and determines that you or your ex was not in a psychological or spiritual state to enter into a marriage and thus spiritually speaking the marriage was invalid. The fact that she had to get an annulment means she probably was married in church but other circumstances indicate that the marrige was wrong in the first place.
So yes, her marriage to Keith Urban will be her first marrige before God.

My cousin married a Catholic girl in the Catholic Church but he had to meet with the priest and agree to raise any children born to them in the Catholic faith. Being as how he is not of any religion, he agreed and the marriage went forward.

The Catholic church only recognizes catholic weddings as valid. Civil divorces are not valid (but they do have a process for annulments of Catholic weddings). Any civil wedding is considered the same as 'living in sin.'
