For example, Antiviruses like Norton add entries to the menu. How can I add my own, or remove Norton's?

I dont think you can, those are standard options on the desktop menu.

I know you can add more "open with" options in folder right click menu but you cant add more options to the desktop right click menu since the registry in your Windows is set up that way.

Windows XP is not open source. So you cannot do that. Well it also depends on your applications you install.
