It depends on 2 things:
1. With what was it pinched?
2. How hard was it?
If it was pinched with a really huge cutting plier such that it was ripped apart, then you could die of bleeding a lot :-)

No, you wont die from it but it will traumatize your preast tissue. Neurologist/neurosurgeons in the hsopital pinches nipples of patient to test for their level of conciousness. This practices is now being discouraged since it's very traumatic, but it is still being done. Constantly pinching your breast and constantly traumatizing it, there's a good chance of developing cancer.
