It looks nonchalant. Like "I could care less that I'm pulling it out in front of someone else". At least that's why I think they're doing it.
Oh, or it could be for them to have something to aim at.

Personally, sometimes I just have to spit at the same time I have to piss. It's not a necessity, just an added luxury at the urinal. Another bonus is advertisement material I can read while relieving myself. It all adds up to a pretty sweet experience. You should try it sometime.

Maybe they just have to spit on their way to pee. Muchmore sanitary to spit in urinal than on the floor. Right???
