Male and female embryos have not differentiated into gender before the nipple tissue has been programmed into the fetus via DNA. Once the embryo is exposed to testosterone or estrogen then the baby can go on to develop into a gender. Although breast tissue doesn't actually differentiate until puberty, it developed very early as a fetus.
And depending on the guy, men's nipples can be sensitive.
