The moon always "comes out." It's just that sometimes it comes out during the day, since its revolution around the earth is slightly different than the earth's rate of rotation (the moon circles the earth at a different speed than the earth spins around). The phases come from the position of the sun in relation to the moon and earth. The part of the moon that we see is the part that is lit up by the sun, and when it looks like a crescent, that's because the part that we can't see is in the sun's shadow.

Indian legend has it, That Mother Moon was once in love with Father Sun. Mother Moon however, was unfaithful and became ashamed, so once a month there is a time when Mother Moon hides her face from the children of Mother Earth. Hence, what we refer to as the "new Moon."

The moon is a reflection of the sun. If it is lined up perfectly with something inbetween then there is no moon. Otherwise known as a "new moon" If you look out on nights that there is a new moon you won't be able to see one. There is a perfect cycle for the moon. The cycle repeats it's self every 28 days going from new moon to full moon

Yes, the moon has a phase known as the "new moon" where it's completely in shadow.
The moon has so many phases because of the angle the sun has to it when it's visible to us in our night sky.
Go into a darkened room and turn on a single bare light bulb. Hold a basketball in your hand and look at it. See the "crescent basketball?" Same principle only on a much much much larger scale.
