5? Not TOO bad, although I did roll my eyes just a little bit.
Too many cliches. The "I'll love you forever and ever!" is too typical and it sounds a bit clingy (to the point where you almost seem insincere), to be honest (my friend actually had a story about this. A friend of hers was getting desperate to keep his girlfriend so he was all, "I'd die without you!" So she left him and said "There. I just proved you wrong. You're still alive, right?"). Maybe find another angle? There are many ways to express your love without exaggerating too much. I'm sure if you love her, you can think of something more original.
Of course if you're 13, you can send it to her and she will say "oMg daT iS sO sWeEt!" and just !!swoon!! over you. Then you'll hold hands and exchange dollar store promise rings or something. Yay! In that case, it's absolutely perfect.
