fords usually have a screw in the center of the crank; however, there might be a set screw on the side of the crank opposite the hand knob

There is a "C" clip retainer that holds the knob onto the handle. They make a special tool that most auto parts stores carry in the "Help" sections. Slip this tool in behind the window handle and push out this clip. The handle should come right off from there.

You need a door handle remover.....just go to Autozone or Checker and ask for one........it will be a thin piece of metal with on end that looks like it would fit between the window handle and the door panel and will go around the part the handle attaches to....just jam it in there and it should pop right off.

The real trick will be getting the door panel off in one piece.

Edit:
Along with what mailbox said.

You need a removal tool. You can get them at any parts store. http://www.sjdiscounttools.com/sgt87600.html

The link is to a site where you can order one, or you can just see the picture to know what you need!
