It depends on what is causing the mutism.

If the vocal cords themselves do not work, then there is silence even in a scream.

If it is the language centers that aren't working well, but the vocal cords do work, then the scream will be just like anyone else's.

if its a trumpet one it has a small end and a big end the small end goes into the bell and it goes higher pitched
