Yes and no.

Dial-Up ISP's will just provide software, you'll need the modem and phone line to get online.

Cable Modem and DSL providers will give, loan, or sell you the Modem, but you'll probably still need a network card for the computer if you don't get an "internal" Cable or DSL modem. Most companies give you Router/Modem combos these days, so you should get a network card for your PC and at least one Cat5 cable.

They will run a cable from the street post to your house (in this instance, I am going to use Cable Broadband, since I have that). They will then run a Coaxial cable into your home, hook it to your modem (also provided by them) and then a cat5/Ethernet cable to your PC. I believe they only provide 1 though, so if you have multiple cables, you may need to purchase more.

ISP stands for Internet Service Provider. When you subscribe with an ISP they provide you with login information to surf the internet and help you set up email if you need to. Most people use Internet Explorer to surf the internet and Outlook Express for email. These programs are standard on all computers.

Yes they do, along with spam blockers, pop-up blockers and all you need to ply the web.
I'm with Earthlink and they have been pretty good, with all those things.
