A psychotic wingnut has an ego centered fantasy supporting their self centered tantrums.

Yes the difference is there.And it's clear if we don't use it to judge others.When we apply definition on others (those we don't like),it becomes a personal attack.
The people with mental illness are thos who are prone to hurt others physically or mentally.
