They will often do a partially nude scene (they're professionals, so they can deal with it).
Sometimes the female is uncomfortable with that, and they actually use a stand-in (where you can't see the face) in any shots involving nudity.
But no, while sometimes the act can be very explicit, it would be extremely rare for a movie couple to actually have sex for real as part of the scene, aside from the porn industry.
I can't even recall any examples of such. I don't think it happens at all.

The only film (outside of porn) I know of that this happened without a doubt was "Bolero" with Bo Derek. Big publicity about it at the time because she did it with the other actor while her husband directed the scene.

They are not really having sex. They are actors. That's why they act it out so good. It would be considered porn, if they were really having sex.
