How to create an OS? Hard work.
You can see examples (with source code) at
http://ecos.sourceware.org/
http://www.rtems.com/
To some, OS means kernal + drivers + gui + other stuff (the full Winders style package). To embedded systems people, it might mean kernal + a few drivers and a good debugger hook. "OS" is kind of a fluid term in that sense.
You can also construct your own customized linux "from parts" and gain excellent experience on what makes an OS. If this interests you, start with this search in google or yahoo:
creating a linux distro

You pretty much don't - not anything more than a primitive minix type of OS.
It has been a long time since one sole person could write an operating system, since the whole term "operating system" has a lot more connotations today than it did when all you had to do was get the disk drive to operate and flash primitive typography onto the monitor.
To even begin thinking about being part of a team that works on an OS, you'd want to do an honours program in Computer Science at a good university. Then after that, if you've gotten really good as a low level programmer, you'd probably want to do study BSD and Linux, and possibly offer up some contributions to those operating systems.

Creating an OS is not an easy task. Not only that you need advanced training in C/C++, assembler and not only, you won't be able to do it by yourself because it's a lot of work...
Examples are not easy to come by on the internet. The only open source OS is Linux and Unix derivates for other system arhitecture. You won't find the sources for windows for example because it's not free!!! (now the same thing is happening to mac os on intel...)

Creating your own OPERATING SYSTEM
is virtually impossible for someone to do alone. Never mind knowing C++/C you would also need to know the computer ASSEMBLY language.
But not to be negative ANYTHING is possible try small programs and work your way from there.
