Sorry but it is six.
Break it down in steps.
Lady 1 shakes hands w/ Ladies 2-4. 3 shakes.
Lady 2 shakes hands w/ Ladies 3,4. 2 shakes.
Lady 3 shakes hands w/ Lady 4. 1 shake.
Everybody's done being polite, total= 6 shakes.

no, it is 6.
the first lady has to shake the hands of 3, then the second has to shake the hands of 2 and the third has to shake the hand of the last. So, 3+2+1= 6

No 6 The 4th lady shook three hands The third lady shook 2 hands and the second lady shook just one hand. The first lady had everyone else shake her hand so she doesn't count.

no thats wrong each person shakes one time
so it would be:
lady one shakes ladies 2,3 and 4
then lady two shakes ladies 3,4
and lastly lady three shakes lady 4 hand
making that 6 hand shakes all together.
