Well it USED to mean that someone was not what was considered the "norm". Like someone with a rainbow mohawk or 72 tattoos or piercings & dressed weird may be considered a freak but now if you call someone a freak it tends to have a sexual connotation to it, like they have sex on their minds constantly

A freak can be a weird person, and/or A GOOD type of freak is someone who like to be kinky in the bedroom, freaky. (fun kind of freak)
