You have to go to court and sign away all rights. She can still persue, but if you sign away all parental rights and refuse to go on the birth certificate that may be an option. You have to check the laws of your state and get a really good fathers rights attorney. But if you participated, you shouldn't leave her to bear the responsibility for both parties. You need to discuss it with her to see what she wants.

Only if another man puts his name on the birth certificate and signs as the father. Or she marries and gets her husband to adopt her child (with your approval of course) Or the baby is put up for adoption. These are the only instances I'm aware of.
