Too many variables!
There are several genres within any genre also.

But, in general, I would have to say a tie with "The hills have eyes" and "Texas chainsaw Massacre"

I seem to remember Amityville Horror literally scared the dumplings out of me. That was a long time ago. And I've never watched it again.

Quite subjectively, a case could be made for Karloff's "Frankenstein," and Lugosi's "Dracula." They came along WAAAAAY before CGI, permissive language codes, or any of the modern "conveniences." Others are scarier, a LOT more violent, often just plain sicker, but the older ones I think offered more, with less.

i,m 34 year old male and the exorcist has alwayz haunted me and the ring part one really f*cked me up. i am a truck driver and i sometimes drive wit the lights on in my sleeper cause sometimes i get the feeling one of those whores are sittn there watching me.

still i will watch one or the other at least one a month. this is the best question posted as of yet

I know people will disagree with me on this but...

There was something very well crafted about that film.
You almost feel like Dee Wallace was really trapped in that car for a week when you watch her. She was so believeable for a moment that I really suspected that the director had been kicking her in the gut to get that kind of reaction... very intense for it's time.

I think it is the Original Exorcist. However, I also think the "sickest" film out there is: Henry: Portrait of a Serial Killer. If you haven't seen it, it's worth the rental.
