The statement is a contradiction in terms.That aside, i think it would depend on the mentality of the person listening to the lie and the mentality and skill of the person telling the lie. Lies are told for lots of reasons.. To preserve dignity, to avoid hurting others...etc etc, sometimes there are not always bad intentions behind it. But, one thing is certain the truth always has a funny way of coming out. So, I'd say it's best to be straight up.

Yes, and the more the number of those telling the lie increases, and the longer this goes on - the more everybody sees it as truth. Finally there will be no one left to doubt it. And believe me - it can take as little as 50 years for a big lie to become truth.

That's basically how brainwashing works.
Truth is a very fragile thing even if the truth is a lie. It is all perception, change the perception and you change the truth...or the lie.
There is no real truth and no real lie. Everything can be bent to see the truth behind the lie and the lie behind the truth. We are mere humans. Contradiction is in our genes.
To answer your question: I have no idea how to tell one from the other anyway.

Some people may believe the lie to be the truth but a lie can never be more that what it is ( a lie).

yes, the story of Christmas for example, i know it's mainly kids that believe it but I'm sure lots of adults do too.
The bible says the Shepards were tending to they're sheep in the fields at the time of Jesus birth. Historians say that in December in Bethlehem it would have been friggin freezing and the sheep wouldn't be out in the fields.

depends if the person lies often and what the lie is.
and if the person being lied 2 is gullible enough 2 believe it. when they press for more details and there is none they should suss.
