It strikes me as being about the classic self-destructive personality; somone who is aware of each of their negative tendancies and still insists on acting them out to the ruination of the relationship with the one person who saw the good in them and tried to save them.
Its pretty bleak stuff, but there is a compelling beauty about self destruction.

Heart the song. Kudos for bringing it up, it's not very mainstream, as far as I can tell. ^_^
As for the interpretation...
I haven't heard it too many times, but to me, it seems like it's from a guy that had someone [ex-girlfriend, I'd assume] that was really kind to him - helped him to become sober and such. And since he was so horrible, he wants her to just hate him and move on; he realizes he doesn't deserve her, etc.
I dunno.
