Show up prepared to remind others about your accomplishments. If possible be ready to measure them with hard numbers (percentage efficiency improved, revenue earned, expenses cut, etc.)
Be honest. Try to be objective about your strengths as well as areas needing growth.
Be mature in your responses to criticism, don't overreact. If you disagree with an assessment, say so but don't get emotional about it.
Above all, walk in with the attitude that you are a valuable asset to the Company.

See what they were hired for, have they completed there job. Did they go above and beyond at any time for the company, review attendace, punctuality have they met all your requirements.
If yes you write up a nice blurb about them and how they are on time work is complete and give them a raise. If any of the company polices or any infractions over the year review them with the employee advise them this is what has prevented them from an increase at this time and keep it moving
