Sit her down, and start like so:

"We've been together for a long time, and I love you very much; together we've accomplished a lot that we couldn't do without each other, and I value your trust and your honesty, and hope you feel the same way."

Then:

"There's something going on with you that I'm worried about, and it's kind of embarrasing to bring up, but I figure that our relationship is solid enough that it shouldn't be a big deal -- lately, your breath has been sour more often than not, and I'm starting to worry that you might have a health problem."

Finally:

"Would you consider going to the doctor/dentist for a checkup, to make sure everything's okay? It's often little signs like this that reveal significant problems, and I'd hate for anything to be wrong."

See? You reassure her that you love her, and you make it about concerns for her health instead of the unpleasantness of her breath. Then, when she gets to the doctor/dentist, either they'll find a problem and cure it, or they'll say "you're fine; perhaps you can cure your breath problem with [some solution]"

There is no nice way about telling her that her breath stinks....but here is a safe way to handle the situation. Tell your wife that its been a while since you and her have been to the dentists. Ask her to schedule an appointment for the both of you to get a cleaning....that way she will never know that it was really intended for her, but that you were just doing a routine visit. Go with her to the dentists and hopefully you can tell the dentists what your problem is and say that your wife has the same problem as well. Hopefully the dentists would get your hint and resolve this problem for you. Also make sure you stay persistant with the check ups .... once every 6 months.

Offer gum or a mint. If this is a close friend, say, I think you need this and hand over the mint. Say nothing more. See if the mint helps. There are people who have bad breath that isn't cured by brushing, flossing, and gargling. They may need to see the doctor or dentist because they have some sort of infection. I had a boss who had terrible coffee breath and would come over really close and almost whisper when he talked to me in the morning. I just stopped breathing and smiled. I'm more confident and older now and would have offered the mint.

That's a delicate situation! You're already coming at it from the right direction. It's something you ought to tell her, but you need to tell her carefully.

Try telling her you need to talk to her, but tell her it's nothing serious, just something that's on your mind. Do this when the two of you are alone and it's quiet, maybe in the evening.

Try something like, "Honey, this isn't a big deal, but it's something I thought you should be aware of. I've noticed lately that your breath just isn't as fresh as it's been in the past. I don't know what's causing it, and I'm sure there are simple ways to rectify the situation, but I thought you might like to know."

If necessary, follow up with plenty of "I love you" and "don't worry about it". ;) Good luck!

I use this in the classroom with my students...keep altoids on you or in the house. You can simply offer her a mint to help her breath.
Or, depending on how sensitive she is, you could make a joke of it, and say something like, "honey, you need a tic tac!"
If its a chronic problem, and her hygiene isn't an issue, maybe she should see a doctor. If she's dieting, that can change the odor of the breath.

You two are husband and wife things like this will come up be honest with her just tell her like this say I think we need to change our tooth paste or mouth wash because it doesn't seem to be working for me then be like I don't wanna be around u or anyone else and have bad breath hint around to her

I had terrible bad breath for years and could not find a solution. The answer came when I retired and suddenly it cleared up. The answer was a diet change. Non-fat yogurt eaten often was the only change in my diet.

hand her a toothbrush and tooth paste and tell her her breath stinks and she needs to brush. she will get over it. and if your a real man then you wouldnt worry about what she thinks about you trying to help her hygene improve.
some ppl just need a blunt way of being told things.

Just tell her. My husband and I have no problems with telling each other that. If you don't want to hurt her feelings going for a stick of gum and offer her one or a mint or something.

no need to tell her, just plan a dental apt. for the family, and when you get there let him know her breath stinks, he should be able to help.
