This situation depends on your contract with your employer. Usually it is spelled out. Understand that if you are under doctors orders to stay home, and they insist you work, and something bad happens....that creates a tort. You could sue them and win.
So, to prevent this from happening, various employers have strict rules about this sort of thing. Most will not allow employees to work if they don't have medical clearance (ie a doctor says its not ok to work there). Ask your boss's boss. Likely, your boss is doing something against company policy.

Well, it depends on what your company says. If you have a doctor's note explaining that you have to take off-that it is mandatory, then your employer cannot fire you. If they do because they feel your doctor's note is not the paper it is written on, well, that is called a lawsuit. Especially if it over exceeds your sick time.
Technically, you are "sick" so you have to use your sick days, that is what they are there for. If the doctor says you cannot go to work, and it goes over the amount of sick days you have, they cannot fire you. You have a legitimate excuse. So, they are not vacation time. They are sick time. courtesy of the doctor and his note.

It's up to your employer. Some employer's require you to use sick/vacation days. Some only make you use those if you have too many days off. Best to check employer policies first. A doctor's note does not supercede policies.

Yes, your employer can make you use sick and/or vacation days when you miss work.

Yes, they can make you use any and all days you have but in order or relevevance - so sick days would come first. Even if you have slip and run out of days, they may lay you off eventually and that is legal.

I work in HR, and you are required to use sick/vacation days to cover your sick time if you want to get paid. If you're off for a significant amount of time and have paid for it, you could use disability insurance (AFLAC, for example) to pay you for your time off. But that doesn't kick in for about a week after you have been off, and you have to be off for a minimum amount of time (it varies depending upon the plan and your company). But no, your company is not required to pay you for any time off because of illness unless you have the sick/vacation days to cover the time.

Of course you need to use your time off. It's not your employer's reponsibility to give you free time off. The doctor's note tells the employer why you are off and keeps the employer from firing you.

It depends on your employer's policy. Some companies require you to use your sick days before you can use FMLA.

That is why the company has sick days. You're damn lucky to have them, because you wouldn't get paid otherwise.
