Because he sucked. That was part of the tragedy of it. He was a weakling. He was not as smart as Michael or as strong as Sonny.
He partied too much and trusted the wrong people.

Fredo, though it never says it in book or movie, appears to have some learningn disabilities or mild retardation. He has a child-like innoncence which causes him to lack the strength and meanness of both michael and sonny. Clearly he does not have the intelligence that Micahel has to lead the family business.

Fredo didn't ahve Mikey's brains and was not a "whatever it take" person. Also he did some bad thing in his personal life that lost him his father's favor.

Fredo was guarding his father when he was shot...he dropped his gun and the assasins didn't even shoot him. He wasn't even a threat.
