Yes we all need it a little bit. because without an incline our calf muscels get less work (the belt shoots the leg back otherwise)

I use the incline but I like to vary it. The treadmill I use at the gym has a hill setting where it automatically goes to 1% then down to 0% then up to 3% then down to 1% then up to 5% etc until it gets to 15%. It is a monster work out that always tires me out. I don think that treadmills are less healthy than running outside though. The surface of the treadmill is a little padded and usually hurts my knees after a long run.

Yes, you could incline the treadmill to maximize your running workouts and gain more endurance.

I never thought to use incline, but for long distacne runners incline can become a major factor. Use it!

I think you should run outside... it is better for you, you can be more social, and it's sooo much less boring! That's just my opinion though!
