It works with the alternator. When you use more accessories, the VR senses the voltage drop and increases the demand on the alternator (which increases engine drag) to supply the accessories.

I can not say for sure about a voltage regulator in a car, but in electronics a voltage regulator is a component that stabalizes voltages with respect to ground. That is to say that if the input to the regulator goes above lets say 12volts the output of the regulator will remain a constant 12volts. Voltage regulators typically have a minimum voltage and a maximum rating. Providing these ratings are not exceeded the output will remain the same.
