Unfortunately, Gerry Baby Products is out of business. So there is no website to check. They were acquired by Evenflo in 1997.
You may want to contact them to see if they might know since they ALSO make (or made) a crib with the same model number.

didnt you get one when u bought it? if not then you can return the box and buy another one with the instructions.
