Is there a website where I can get a form or something? I have no idea what the ID number is.

You would probably have to go to your DMV with some other from of ID like a birth Certificate and utility bills or SS card..I don't know if you can do it on the website.

You have to physically go into the place you first got it and get a replacement. In most states it's the same place that issues the driver's licenses.

Go to the non-DMV? Seriously, just go to the DMV, they should be able to make you a new one. Just call first to be sure what kind of ID they need.

go to the motor vehicle registry and bring a couple of ids and you will get one check the motor vehicle for your state online to see the cost

quickly report to local DMV, they'll give you a brand new one with some fees...
