One of the neatest invitations I received was for a surprise going away party for a friend in college. Keeping with the "good luck" theme, the hosts bought a bunch of fortune cookies. They carefully pulled the fortunes out and replaced with their own message. They then put the cookie in a small box with an instruction sheet telling the recipient to open the cookie and read the message. They mailed many of the invitations and local ones they hand delivered. Once opened, the fortune unfolded and revealed the invitation. (Very tiny font) Everyone was asked to bring a "good luck" item to the party for the guest and to be there at least 30 minutes early. They carried the Chinese theme throughout the party.

If you're planning to buy invitations, they have them in the stores "Surprise Party", then of course just fill in your blanks.
If you going to make them on the computer, other than the date, time, place, etc. you put exactly what you stated in you question.
It's a surprise party to say goodbye and good luck to friends who are moving.
