I dont get it lol... ive heard this metaphor so many times. but I just dont get the true meaning behind it :)

It means that you aren't afraid to show emotion. And you aren't shy about who sees it. I guess the expression means that emotion comes from the heart... and by wearing your heart on your sleeve ; you express your emotions... where anybody can see them [ on your sleeve ].
