Once every million miles. Ford has a maintenance schedule on it, but usually never. These days the rear ends are having lots of problems though. Soft metals. If it's making nasty noises (growling), make them fix it. My dad has rebuilt dozens of them in the past year. On various size Ford vehicles. If it's a newer Ford and you've already torn it apart, there's an additive for it that you don't want to forget. Smells like whale oil...probably is. Nasty stuff.

Unless it becomes submerged in water or some other similar instance. I would say never to once maybe twice in the truck's lifetime. I've seen gear sets that have 40 year's worth of use on the original fluid, which show virtually no signs of wear on the ring and pinion or bearings . Just check it periodically for leaks and check the fluid level once in a while.

never flush,just change fluids every 30,000 miles.
