It's a throw back to the days before yellow and red cards. To record bad fouls or other offensive behaviour, the referee used to just write the offending player's name in a book (he still does) but not show a card - hence the term "booking". Now we have the card system (introduced in 1976) so a booking is accompanied by a yellow card.

You committed a cardable foul or offence and the referee showed you the yellow or ed card. Then he writes your number down in his book, hence the term "booked."

Booked means yellow card
send off - red card.
2 yellow cards = red card.

It means you get a yellow card! (remember, 2 yellows in one game gets you kicked out and usually suspended for the next)
