Good question!
First of all, in the the soccer World Cup (very unique competition) it is very difficult to make predictions just based on previous games or from individual biased opinion (meaning fans).
Yet, the analysis of the game can be based on few other factors that may or may not result truthful to the game: condition of the players, player's availability and coaches’ strategy.
The Czech Republic's performance with Ghana was (to be nice) embarrassing: players were not able to keep the pace of the Ghanian players and the whole team seemed too predictable.
Similar limitation was showed by Italy in the game with the USA, but the Italians have the justification of having in front of them a Team (USA) that was very aggressive and physical.
The Czech Republic most likely will be able to have Milan Baros starting the game, but the absence of Jan Koller, Tomas Ujfalusi and Vratislav Lokvenc is a big lost for the Team, the substitute are not at the same level of the above players.
Italy will not have De Rossi, and maybe Iaquinta, but they have plenty of players available for the same position.
Karel Bruckner most likely will play with Barros first forward with Rosiky, Nedved and Poborsky ready to alternate as a support for the attack. Not having available Ujfalusi, the defense will be weaken and with not so much effect on "high balls", where the Italians may be very dangerous (Toni and Gilardino)
Marcello Lippi has not uncovered the initial formation: it may be the same used in the two previous games (with Gattuso in De Rossi's place and Grosso in Zaccardo's place) or he may decide for a more solid and conservative 4-4-2, with Toni-Glardino as forwards, Totti on the bench and Camoranesi in the middle with Gattuso, Pirlo and Perrotta. As second on how the game will develop, Totti, Del Piero, Inzaghi and Iaquinta may be utilized in the second half.
My prediction is that Italy has surely bigger chances of winning the game, considering another factor mentioned by other answers too: the motivations of the Italians in the key games transform them to an unbeatable Team.

The game will be close although i think Czech Republic will be down on themselves for being upset by Ghana and Italy ready to go coming out of a hard fought and physical game against US.

Italy will not underestimate the Czech Republic like they did the US team. Also, Italy needs to win to avoid Brazil in the knock-out stage.

Well, both these teams have a great deal of respect for each other and know each others game well, for example Nedved plays an important role fro Juve. When it comes down to it, The Azzurri is the better pick just by the amount of experience and talent that team has. Italy wont win by alot but look for a 1-0 or 2-1 win for italia!! So don't sweat it Juventina!!

i think Italy is going to beat Czech Republic if they don't underestimate this team as they did with the USA so gooooooo Italy go Totti,Luca Toni, Geraldinio,Delpiero,Nesta,
