When I was doing my research paper on Music Censorship I came across an album cover that was very controversial, I don't remember the band but the cover had a pile of garbage, like at a landfill or dump. But in the pile of garbage there were human fetuses.

If you are talking mainstream artists, John Lennon/Yoko Ono on the two virgins album were completely frontally nude. Blind Faiths album that showed Ginger Bakers 12 year old daughters breasts is another one that comes to mind. If you want to go off the beaten path, any one of Cannibal Corpses album covers will fall into this catagory. Those guys have a couple that will make you nearly vomit.
