Actually all of the other answerers missed one- Czech Republic , they have played as Czechoslovakia. Here's the list-
Europe- Ukraine, Czech Republic, Serbia & Montenegro
Africa - Ivory Coast, Ghana, Angola, Togo
Concacaf- Trinidad & Tobago
ps- Paraguay have played many times before. Australia in 1974.

What a question!!
In Africa there four of the five teams who are at this year's world cup are new comers. These are Ivory Coast, Togo, Angola and Ghana, in the order of elimination.
In Europe, the only new comer is Ukraine. Also in the South America has got its new comer, Trinidad and Tobago. All the other teams have been there before.
Thus, there are six new comers in this year's world cup;
Ivory Coast
Togo
Angola
Ukraine
Trinida and Tobago
Ghana
