You know I have similar thoughts when I lose at gambling, "did I calculate the probability's right? did I concentrate too much? Did I do anything wrong", but I think the truth is that sometimes it just comes down to luck. Maybe you were just unlucky that the things you remembered were not the things asked in your exam. Its a shame, but luck does come into everyday life quite often and of course its often bad luck.

You should never cram, but it sounds like you didn't. Memorization is important, but you also just have to know certain things. When a "blank out" occurs and you suddenly forget everything you studied, think about something else for a few seconds and then come back to it. Even if you don't know the answer, write what you do know and you could get some partial credit (if your school gives this)

yep.. it's possible... It's way better to study like a day or two before the exam. Then your brain will get enough rest and you get your best score for the test.
As for me... usually a day before test I just stop studying and relax.. Have sone fun)

well, you prabably went into a stage kinda like shock. You got nervous and your brain can in a sense, 'shut down.' But, overloading it can also cause it.

your first ten minutes of studying is the most productive. you stop truly learning after an hour--after that point it's a gamble what short-term facts you will and will not remember.

maybe you just blanked from the pressure, that happens to alot of people, they're just not good at taking tests...or maybe your professor's outline was off, I had one professor whose test was the complete opposite of the review, it threw everybody off...and that's just mean:P
