Here are all of Undertaker's vacations:
1) At the 1994 Royal Rumble, he lost a casket match to Yokozuna and returned at SummerSlam later that year. He actually took time off to heal a back injury.
2) At the 1996 In Your House: Buried Alive, he lost a Buried Alive match to Mankind and came back at the Survivor Series the next month in a more Satanic gimmick. This isn't a vacation so much as a retooling of his gimmick, but the implication at the end of Buried Alive was similar to that of his previous "death".
3) In the fall of 1999, Undertaker was trying to help Big Show discover his "dark side", but he up and quit on Big Show one day and didn't return until Judgment Day 2000 with his "American Badass" gimmick. He took off time then to heal a groin injury and get married.
4) At Survivor Series 2003, he lost a Buried Alive match to Vince McMahon with the help of Kane and didn't return until Wrestlemania XX in 2004 where he fused his old and new gimmicks together and appeared with Paul Bearer to defeat Kane once again. I don't think there was an explanation given for this time off, although I have heard that the more recent "deaths" are to make Undertaker's appearances more of a big-time event when he actually does appear.
Currently, he's supposed to be at home helping out after his second daughter was born just after Wrestlemania 22.

Its a publicity thing, I think. It gets people all aggravated and stuff then they're ready to rumble when they find out hes back kind of like in the soaps when they kill people off but then you see them again because they weren't dead just kidnapped or something.

i think hes actually died a total of like 3 times
they dont kill him every time he leaves
sometimes they just fake like hes hurt then he heals up and comes back
